Most access equipment that fails does not warn anyone first. When that certification has lapsed, the machine has not necessarily become dangerous overnight, but nobody can honestly say it has not. It has simply become an unknown.
That distinction matters because the parts of an elevating work platform that decide whether it holds together are mostly out of sight. Without a current, competent assessment, an owner is trusting the machine on faith, and faith is not a defence when a platform is carrying someone at height.
Where a crack begins
Structural failure in a boom rarely starts as a break. It starts as a microscopic crack at a point where stress naturally concentrates — the toe of a weld, a boom pivot, a bolt hole, or a change in section. Every time the machine is raised, slewed, driven or loaded, those points flex a tiny amount, and over thousands of such cycles the crack grows a fraction at a time.
This is fatigue, and it is unforgiving precisely because it is gradual. The steel can look completely sound while a crack quietly extends beneath the surface or under a coat of paint. There is no gradual sag, no obvious groan, no leak to tip anyone off. The member is intact until the remaining metal can no longer carry the load, and then it separates.
Why a walk-around cannot find it
Operators are right to check their machine before use, but a visual pre-start was never designed to detect fatigue cracking. The most dangerous cracks are sub-surface, hidden inside box sections, or masked by the very paint that is supposed to protect the steel. The eye cannot see them, and a machine that passes a glance can still be days of work from failure.
This is the entire reason the inspection regime exists in layers and culminates in a deep major inspection — carried out at five years or ten years depending on the equipment class — where critical welds and members are exposed and subjected to non-destructive testing. What deferred maintenance quietly does
Putting off maintenance rarely feels like a decision at the time. A servicing slips, then another, and the machine keeps working, which seems to prove nothing was wrong. What is actually happening is that small problems are being allowed to compound.

- Corrosion eating into load-bearing members and creating the sharp pits where fatigue cracks prefer to start.
- Worn pins and bushes introducing play that raises the loads passing through structural joints.

The person is at the top of a machine that has just lost the thing holding them up, and it happens without warning in the time it takes for the last of the sound metal to give way.
Every layer of certification and inspection exists to keep that scenario from ever arriving. An uncertified platform removes the layer that was specifically meant to find the fault before the person was ever standing on it.
The regulator and the duty you carry
