The Dangers of Running an Uncertified EWP in Western Australia

Why a cracked, unmaintained or out-of-certification elevating work platform is a real threat to life on Western Australian sites, and what the failure means for the business behind it.

Western Australia asks a lot of its access machines. The distances are enormous, the remote work is unforgiving, and a platform that develops a fault a long way from a workshop is a platform that too often keeps working. That is exactly the wrong instinct, because the faults that matter most on an elevating work platform are the ones nobody can see — the quiet cracks and creeping wear that turn a routine lift into a fall. The one failure mode you cannot walk away from

Most equipment gives you a chance. It stumbles, it warns, it fails somewhere you can stand clear. An elevating work platform, when its structure fails, does none of that. The boom or basket parts suddenly, the platform drops, and the operator falls from height with no warning and nowhere to go. The very thing the machine is for — putting a person up high — is what makes its failure so serious.

That is the reason access plant is governed the way it is. You cannot manage this risk after the fact, because there is no after the fact for the person who was in the basket. The whole system of inspection and certification exists to catch the fault while it is still a small crack in a workshop rather than a serious incident in the field. Remote or not, that logic does not bend to distance.

Fatigue cracking, explained without the drama

A fatigue crack is not the product of a single violent event. It is the product of repetition. Every time a boom lifts, extends, slews and lowers, the structure feels a stress cycle, and the points that concentrate that stress — weld toes, boom pivots, section changes, holes — take the brunt of it. After enough cycles at perfectly ordinary loads, a crack initiates at one of those points and begins to lengthen, a fraction at a time, with every cycle thereafter.

The danger is in how long this stays hidden. A crack can grow for a very long time beneath paint and grease, invisible from the platform and invisible from the ground, while the machine behaves exactly as it always has. In WA that is compounded by hard, high-cycle duty on remote work, which stacks up fatigue faster than the machine's age would suggest. Nothing an operator can do on a walk-around will reveal a crack that has not yet broken the surface — and by the time it does, the metal has usually been coasting on a shrinking margin for a while.

What deferred maintenance takes away, piece by piece

Skipping maintenance does not freeze a machine in good condition; it lets the machine spend down the safety it was built with. The effects accumulate quietly across both the structure and the hydraulics.

Left undone, maintenance lets the following build unseen:

  • Corrosion at weld toes and structural members, removing metal and sharpening the very notch a fatigue crack needs to start.
  • Worn pins and bushes that let boom joints develop play, and slew-ring bolts that loosen until the turntable rocks and pounds.
  • Hydraulic decline — split-prone hoses, worn seals that let a raised platform drift down, and holding valves that stop locking the boom in place.
  • A dwindling paper trail, so the small faults a competent inspection would have caught are never recorded and never fixed.

Without current certification, the machine is a question mark

The failures most likely to cause serious injury are hidden inside the structure and the load-holding systems, where no operator can look. Certification is the mechanism that puts a competent person's eyes on those hidden parts and records a verdict. Under AS 2550.10 that verdict is built from a layered regime — routine and periodic checks, whose spacing is set by the manual, the workload and a competent person rather than any fixed interval, sitting beneath the deep major inspection.

The major inspection is the strip-down that deliberately searches for the hidden fatigue cracking described earlier, and its timing is not a single figure for everything: it is due at five years or at ten years depending on the equipment class, counted from the machine's manufacture. Once certification lapses, so does the only reliable statement about the machine's concealed condition. An uncertified boom is not a boom presumed good — it is a boom nobody has vouched for, and every lift on it is an unanswered question.

The regulator and the duty you carry in WA

Western Australia moved onto the harmonised work-health-and-safety framework in 2022, with WorkSafe WA as the state regulator. The primary duty falls on the person conducting a business or undertaking to ensure, so far as is reasonably practicable, that plant such as an EWP is safe. Maintaining, inspecting and certifying a machine is not something separate from that duty — it is the way the duty is met.

If a machine fails seriously, WorkSafe WA investigates, and the enquiry turns on evidence: the last competent inspection, the maintenance record, proof the unit was fit for service. Given how much WA work happens far from a depot, the temptation to defer is real — but a platform whose condition cannot be shown is a platform whose owner cannot demonstrate the duty was met, and remoteness is not a defence for it.

What a failure means for the business

The human cost aside, a serious failure hits the business hard and on several fronts. The machine is stood down at once and its history examined, the work stops, and a mechanical failure becomes a broader judgement about how the business runs its plant — often a long way from where the machine sits.

The consequences reach into commercial relationships too. Principal contractors expect evidence that the equipment was compliant, and an insurer assessing a claim will look closely at whether the plant was maintained and certified when it failed. Rebuilding that trust after an incident is far harder than keeping it.
